The pressures of adolescence, families, and fashion can send some down the path toward bulimia or anorexia, which holds the highest mortality rate of psychological disorders. On the other hand, pro-ana voyeurs get off on the images of woman wasting away. Tiffany Philips examines eating disorders in the Czech Republic.

Five beautiful, slim teenagers sit in the Palac Flora food court, where McDonald’s cheeseburger value meals and milkshakes are the lunches for three. I grab a latte and head for a table nearby, noting the other two aren’t eating. I observe them for about 30 minutes. Suddenly, in typical female fashion, they stand, heading for the toilets. I follow them and push open the door, heart pounding as I am greeted by the sounds of gagging and choking, followed by chunks of vomit plunking into the water.
Many factors provoke males and females of all ages to resort to such grotesque measures while attempting to look perfect. “Environmental factors, genetics as well, play a significant role in distorted self-images garnered by anorexics and bulimics,” says Dr. Martin Hollý, psychiatrist and sexologist at Bohnice Psychiatric Hospital, “A large majority of patients come to us with problematic relations with their mothers. Whether the pressure to please, or a young girl’s way of unconsciously crying out for attention, they crave a sense of control and find it by restricting their food intake. Accordingly, girls follow the success of being ultra-thin, a ‘trend’ glorified by the media.”
The fashion industry receives the brunt of criticism, relating to the amount of pressure it thrusts upon young women to be thin. Major design houses—Gucci, Dior, Louis Vuitton, and others—set the standard for high street fashion. Producing small sizes with limited supply, designers ensure only Barbie-sized women will represent their brands.

Researchers estimate 70 million people worldwide have eating disorders, including anorexia, bulimia, and binge-purge eating. In the Czech Republic, approximately 5 to 10 percent of the population reportedly suffers from one or more. “There has been a significant rise in eating disorders since the [1989] political fall,” says Dr. Hana Papežová, head of the Eating Disorders Unit at Charles University Medical School. “There was a diet craze, definitely, although we are unaware of how steep the numbers really are.” She estimates her unit treats approximately 200 inpatients, 30 to 40 more at the day-care center, 50 to 60 outpatients, and many chronic cases that require year-round treatment within the facilities. Most patients are vegetarian, using indulgent methods to control their weights, including diet pills, laxatives, and other diuretics, but most often, starvation. In men, steroid use and compulsive exercise are the most common forms of manipulation.  
Gail Whitmore, an American counselor residing in Prague, treats 10 patients with eating disorders at any given time. “As soon as one leaves, another comes,” she says. “I have patients from all over. Girls come here to live, and it is especially hard because Czech women are so thin, genetically thin. They leave home to escape something, but lose their support system in the process. In most cases, patients become detached and isolated from family, friends, and activities they enjoy.”  Markéta, a student, offered insight into her ongoing battle with weight control: “I am ashamed, always ashamed ... and alone.”  
ImageNot all suffering from eating disorders are ashamed and alone. Anorexia, which holds the highest mortality rate of all psychological disorders, has an active group of supporters: the “pro-ana” community. They set to inspire girls to be more creatively self-destructive, with the belief that anorexia is a lifestyle choice, not a medical condition. Countless Web sites pay tribute to anas, using various forms of expression, including anorexic porn. “The pro-anas are like a really bad religion,” Dr. Papežová says. “It’s shocking they encourage other girls to abuse themselves, as well as offering pornographic images to motivate them to be as thin as possible.” Available to anyone who spends a few minutes browsing the Web, images depict the gaunt, smiling faces of emaciated, decomposing male and female  forms, every vein and bone visible.  If these men and women were displayed on posters for those starving in third-world countries, people would be sending them food, rather than supporting their disillusioned vanity.

Poruchy v příjmu potravy u občanů v České republice

Pět mladých atraktivních dívek sedí v občerstvení Paláce Flory, ve kterém se jedna porce McDonalda spíše podobá obědu pro tři. Objednám si kafe a zamířím k sousednímu stolu, když si povšimnu, že dvě dívky nic nejí. Po třicetiminutách najednou vstanou a zamíří na toalety. Následuji je a s bušícím srdcem otevřu dveře, abych uslyšela zvuky žblunkání kusů zvratek do vody záchodů.


ImagePodle psychiatra a sexuologa Bohnické psychiatrické léčebny Martin Holého, je důvodů k podobným aktivitám více. Svůj podíl na deformaci představy o sobě samém nese nejen okolní prostředí, ale také genetika. Častým úkazem u takto postižených dívek bývá touha po pocitu vlastní kontroly, kterou uskutečňují omezením příjmu potravy.


Průzkumy odhadují počet lidí trpící poruchami příjmu potravy, včetně anorexie a bulimie, až na 70 milionů. V České republice se podle MUDr. Hany Papežové, počet nemocných po politické změně v 89’ zvýšil na 5 až 10 procent y celkové populace. Její oddělení, které je součástí 1.lékařské fakulty Karlovy Univerzity, léčí kolem dvouset stálých pacientů, 30 až 40 pacientů docházejících, 50 až 60 externích pacientů a mnoho dalších chronických případů které vyžadují celoroční péči. Většinu pacientů tvoří vegetariáni kontrolující svoji váhu metodami zahrnujícími prášky na hubnutí, projímadla, a nejčastěji hladovění. Muži nejčastěji inklinují k užívání steroidů a přehnanému cvičení.


Přestože se většina pacientů postupem času ocitne v osamocení a odloučení od rodiny, přátel a oblíbených aktivit, najdou se i takoví, kteří samotou či hanbou netrpí. Anorexie, psychická porucha s nejvyšší úmrtností, má svojí vlastní skupinu aktivních stoupenců: tzv. ‘pro-ana’ společenství. Jejich cílem je vytvoření inspirace pro ostatní dívky k více tvůrčím metodám sebedestrukce, vycházející z přesvědčení, že anorexie není somatický stav, nýbrž volba životního stylu. K dispozici je řada webových stránek, včetně anorexické pornografie, ctící toto ana hnutí. “Seskupenství ‘pro-ana’ je něco jako zkažené náboženství”, vysvětluje MUDr.Papežová. “Přijde mi to šokující, podporovat jiné dívky k sebezneužívání, a to navíc využíváním pornografických materialů za účelem motivace k jejich hubnutí”.
